  • Area XV Agency on Aging Seeking Vendors
Agencies and organizations are being sought to provide services for the elderly and disabled persons, in home.

Services will be brokered through Hoosier Uplands under the CHOICE, Medicaid Waiver, and Social Services Block Grant Programs.

Services needed are transportation, respite care, homemaking, home health care, home delivered meals, adult day care, and attendant care (sitter).  Services are reimbursed on a vouching basis after delivery services.
