  • Licenses and Accreditation

Home Health Care was originally certified by the Indiana State Department of Home Health in 1977 and accredited by CHAP in 1992.  Hospice was certified and accredited in 1996.  CHAP and the State Department of Health make unannounced visits to assure quality standards are being met.

Home Health Care and Hospice are fully licensed by the Indiana State Department of Health, certified to provide Medicare/Medicaid services.  Both also provide care through the Social Services Block Grant (SSBG), CHOICE, MAW (Medicaid Waiver), private pay, veterans administration, and commercial insurance.

     CHAP Accreditation

Hoosier Uplands Home Health Care has been serving the needs of Southern Indiana since 1977.  Hoosier Uplands Home Health Care has been accredited by the Community Health Accreditation Program (CHAP) since 1994.  To earn the CHAP "Gold Seal of Approval," home health care and hospice agencies must demonstrate they continuously strive to improve client care.  A CHAP accredited organization meets the highest quality standards in the nation.  This means Hoosier Uplands Home Health Care & Hospice has gone above and beyond what is required of them by the Center for Medicare and Medicaid Services (CMS) which is the federal agency that regulates home health care & hospices across the country.

Hoosier Uplands Hospice began serving the needs of Southern Indiana in 1996.  Hoosier Uplands Hospice has been CHAP accredited since 1999.  In 2002 Hoosier Uplands Hospice became the first Deemed Status Accredited Hospice in the State of Indiana.

 

At the exit conference Roseanne Bordenkecher, RN, MSN and Johanna Dall'au, RN, who were the CHAP Surveyors, had very good comments regarding Hoosier Uplands Home Health Care & Hospice.  In regards to Hoosier Uplands Hospice they stated, "The patients were very pleased with services and complimentary of staff.  The care given to the patients is obviously good."  In regards to Hoosier Uplands Home Health Care they stated, "The patients went out of their way to compliment the staff.  Your staff are to be commended."  In regards to the agency in general they stated, "It is unusual to have the longevity that Hoosier Uplands has.  It is apparent the staff care about the patients and their job."    

  • Home Health Care and Hospice Services

          Home Health Care and Hospice are vital services, especially in the rural areas

          served by Hoosier Uplands.  Services provided by the home care staff can

          prevent or shorten a hospital or nursing home stay.  Home Health promotes

          independent living and speeds the rehabilitation and recovery process by

          allowing a person to recuperate in familiar home surroundings.

 

          Hospice supports the client and family as they face the last stages of life, by

          offering support and comfort, primarily in the home setting, allowing

          dignity, self control, symptom management, and family togetherness.

 

          Hoosier Uplands Home Health Care & Hospice is committed to providing

          the highest quality and most cost effective home care in its service area.  Our

          focus is on patient satisfaction.  We recognize and emphasize the critical

          concept of provider integration, working as a team with physicians, hospitals,

          nursing homes, managed care organizations, as well as public and private

          insurers, in order to deliver health care at home.

 

          Services are available 24 hours a day, seven days a week, including holidays. 

          Outside of regular office hours the answering service contacts the RN or

          administrator on call.

    Home Health Philosophy & Purpose Statement

    Philosophy:  Hoosier Uplands Home Health Care is committed to providing

      quality home health services to eligible persons in Lawrence, Orange,

      Washington, Crawford, Harrison, Scott, Martin, Dubois, Jackson and Clark

      counties.  The health care is delivered at reasonable cost, regardless of race, creed,

      color, sex, age, handicap, sexual orientation, veteran status, or lifestyle.  Health 

      promotion, maintenance, education, and coordination for continuity of care are

      utilized in a comprehensive approach to the client, family, and community.

 

      Purpose:  The primary purpose of Hoosier Uplands Home Health Care is to

      provide quality home health care services to residents within the service area.

      Nursing services, physical and occupational therapy, speech language pathology,

      medical social services, and paraprofessional services will be delivered to

      appropriate persons, in need of care in their home, under the direction of

      licensed professional personnel.

  Hospice Philosophy & Purpose Statement

  Hospice Philosophy:  Hospice affirms life.  Hospice exists to provide support

   and care for persons in the last phases of incurable illness so that they might live

   as fully and comfortable as possible.  Hospice recognizes dying as a normal

   process.  Hospice neither hastens nor postpones death.  Hospice exists in the

   hope and belief that through appropriate care and the promotion of a caring

   community, sensitive to their needs, patients and families may be free to attain

   a degree of mental spiritual preparation for death that is satisfactory to them.

 

   Hospice Purpose:  Affirming the hospice philosophy, our purpose is to provide

   a comprehensive program of care and services to the terminally ill and their

   families, enhancing their quality of life through the management of physical,

   emotional, social and spiritual needs, which accompany terminal illness, death,

   and bereavement.  To provide quality hospice care and be an integral part in

   developing, promoting and educating the public about hospice care and its role

   in the healthcare system.  To be a key member of the health care delivery

   system in Lawrence, Orange, Washington, Crawford, Harrison, Scott, Martin,

   Dubois, Jackson and Clark counties, in order to meet the needs of the

   terminally ill and their families, as a medicare/medicaid certified and CHAP

   accredited provider.  To strive for excellence by educating physicians, social

   workers, clergy, allied healthcare personnel, paraprofessionals, and volunteers

   regarding the care of the terminally ill.
