Opened in March 1999 under Nurse Managed Clinic Funding from ISDH.
Funding for Operations Sub-Contracted to Memorial Hospital and Health Care Center (of Jasper).
Serves as a Rural Health Clinic and Health Care Professional's Rural Training Site.
Originally housed in the Shoals Elementary School Nurse's office, the clinic relocated to a newly built facility in Shoals owned by Dr. Kerr which houses office in 2001.
Hoosier Uplands manages the following funding relating to this clinic: Community Health Center Funding ($90,000: July 2007 - June 2008)
Greg Mahuron serves as the project's primary point of contact.
