- Patoka Family Health Care Center
The Patoka Family Health Care Center opened in March 1997 under Nurse Managed Clinic Funding from ISDH. The funding for operations of the clinic is sub-contracted to Southern Indiana Community Health Care, Inc. This clinic serves as a Community Health Center, Health Care Professional's Rural Training site and a Rural Health Clinic. It also participates in the Primary Eye/Optometry services pilot project with ISDH and Indiana University School of Optometry.
In 2001, Hoosier Uplands was awarded $380,000 from Indiana's share of the Tobacco Settlement to build a new facility in Crawford County to house the clinic. Hoosier Uplands currently manages a contract in the amount of $148,000 for this clinic that is effective from July 1, 2008 through June 30, 2009.
