Area XV Agency on Aging - Case Management
Hoosier Uplands Case Managers work with the elderly and disabled to assess their needs and offer options such as in-home care so the person may remain in their home in a safe and feasible environment. Upon determining the need, the Case Manager and client, or family member, will decide which in-home program(s) best fit the client's situation such as CHOICE, Social Services Block Grant (SSBG), Title III or Medicaid Waiver. We have several different waivers which include: Aged & Disabled, Traumatic Brain Injury, Developmentally Disabled, Autism, and Medically Fragile.
Case Management also assist families with the Pre-Admission Screening (PAS) Program. The program assists families in determining whether or not nursing home placement is appropriate and helps with facilitating the placement if necessary.
